Explanation for recent Salesforce email - Update web to case, web to lead endpoints

Salesforce recently sent an email titled "ACTION REQUIRED: Update Web-to-Case, Web-to-Lead, and Salesforce.com endpoints by November 17, 2017". You may have read it and wondered 'what does this mean to me?'.
If you have a custom integration with Salesforce through one of Salesforce’s APIs, you may be impacted. Written in API, there may be a hard-coded reference to www.salesforce.com, and that needs to be changed. If you do nothing, the API will no longer function, and your API calls will result in a “404 (Page Not Found)” error.
Makes perfect sense, right?
Here’s what you really need to do. First, determine if you are even using Salesforce’s API. Many customers use 3rd party applications to handle their integrations, and those vendors will take care of any changes necessary. That is what is so great about a Managed Application! Where this falls on you, is if you have a custom integration set up so that Salesforce and another one of your databases can communicate with one another. In this case, work with your Salesforce Team to first search for this specific wording, and if they find it, update it. See, not so bad.
Web-to-Lead & Web-to-Case
The other change taking place has to do the Web-to-Lead and Web-to-Case functionality.Customers using either or both of these functionalities will need to update the HTML code in their websites by November 17, 2017.To make the HTML code update on the websites, you should provide the following information to your company webmaster:
